Due to the volume of requests for verification of membership and in effort to expedite our response to those queries, the Good Samaritan Hospital Medical Staff has adopted a policy that it will verify membership, dates of affiliation, and staff status for current and past medical staff members using on-line verification. We regret that we do not have the staff to allow us to fill out individualized ratings forms or to provide data on activity and quality assurance review. You may complete your verification by providing the physician's last and first name as well as middle initial. Our affiliation letter can be printed immediately. At times our list is incomplete, and you will find that a physician who is or has been a member of the staff is not listed. If the name is missing, you will be prompted to contact us directly.
